How to obtain a digital signature

  1. Navigate to the CONNECT! Provider Registry at https://connect.clearvalueconsulting.com.
  2. Log in to the site. If you have not previously registered, click Register as a New Provider and follow the wizard. Once registered, log in.
  3. Click the Digital Signature menu item on the left.
  4. Download and save the Signature Template file to your computer. (Also available below, or click here)
  5. Print the document and sign your name in the designated area.
  6. Scan the signed signature template and save as a JPG file format. (Do not save as PDF!)
  7. Upload the signature template by clicking on the Select button.
  8. Click Process Signature button. This will convert your uploaded file to an encrypted signature file
  9. Go to the Documents Tab on the left.

When signing reports in the Acuity Forms Portal, you may be prompted to enter your CONNECT credentials.
