How to obtain a digital signature
- Navigate to the CONNECT! Provider Registry at https://connect.clearvalueconsulting.com.
- Log in to the site. If you have not previously registered, click Register as a New Provider and follow the wizard. Once registered, log in.
- Click the Digital Signature menu item on the left.
- Download and save the Signature Template file to your computer. (Also available below, or click here)
- Print the document and sign your name in the designated area.
- Scan the signed signature template and save as a JPG file format. (Do not save as PDF!)
- Upload the signature template by clicking on the Select button.
- Click Process Signature button. This will convert your uploaded file to an encrypted signature file
- Go to the Documents Tab on the left.
When signing reports in the Acuity Forms Portal, you may be prompted to enter your CONNECT credentials.
